Making a questionnaire for myspace?

sorry if this is confusing..Im looking for a web page that will help me make a poll..I want to make a list of yes or no questions for friends..then be able to see the percent of who said what, but it will be anonymous.(kinda like voting polls). The problem is most sites only let you make one question polls or a survey where everyone can see who wrote what..if I got a little confusing here's an example:

1.Did you ever pretend you were a power ranger:

A.Yes

B.No

2. Did you ever think cooties were real

A.Yes

B.No

.......

Then after they answered they can see the percent of people who said yes or no for each question...Does anyone know a site that will do this

They have an option where you can embed your survey/poll right into your page and show survey/poll takers survey/pll results upon completion.

1) When you create account, create your Survey

2) Choose option "Embed your Survey"

3) put html into your myspace account

4) from Survey options select "Show Results upon completion" option

It is very easy to use. I am a handicap when it comes to computers, and even I was able to figure it out.
